Job description
Choice Health at Home is seeking a Home Health Physical Therapist in Denver, CO for a full-time position. You’ll deliver one-on-one care in patients’ homes to improve mobility and independence, with autonomy and meaningful patient relationships.
You will perform evaluations, design plans, and educate patients and caregivers while collaborating with the care team and maintaining proper documentation.
