Job description
CenterWell Home Health is seeking a dedicated Home Health Physical Therapist to help patients regain strength, mobility and independence in the comfort of their homes, reporting to the Clinical Manager.
You will plan and administer prescribed therapy, assess strengths, develop rehabilitation programs aligned with physician plans, supervise PTAs and aides, document progress and communicate with physicians, the care team and families to support optimal outcomes.
