Job description
Colusamedcenter in Colusa, CA is seeking a dedicated Home Health Physical Therapist to deliver patient-centered care in the home health setting. You will assess, treat, and monitor adult and geriatric patients, coordinating with the Home Health Director of Nursing to ensure high-quality outcomes.
The role requires a current California PT license, BLS certification, and strong communication and organizational skills. Experience in home health and driving to patient homes is preferred.
