Job description
Hueman is seeking Nurse Practitioners to conduct in-home Health Risk Assessments (HRAs) for Medicare/Medicaid members around Lavonia, GA. This role offers flexible scheduling and travel reimbursement to providers who value community connections and patient-centered care.
Nurse Practitioners will perform non-invasive assessments, review medical histories, and document findings for proper care coordination. Training is provided, with liability insurance options available.
