Job description
Source One Management Services, LLC is seeking licensed Nurse Practitioners and Physician Assistants for flexible in-home Health Risk Assessments (HRAs) with Medicare Advantage members in Lockport, NY and surrounding areas. This PRN role emphasizes autonomy, a predictable minimum schedule of 8 days per month, and competitive pay with incentives.
Providers will perform in-home evaluations, review medical history, medications, and social factors, and document findings using a company EMR system.
