Job description
Source One Management Services, LLC is seeking Nurse Practitioners and Physician Assistants to perform in-home Health Risk Assessments for Medicare Advantage members on a flexible PRN basis.
You will review medical histories, current medications, and social factors, perform basic physical and mental health evaluations, and travel locally to patients within roughly 60 minutes of your home. No ongoing treatment responsibilities, strong autonomy, and streamlined documentation with an iPad.
