Job description
Activate Care in Massachusetts is seeking a dedicated Registered Nurse to conduct in-home assessments for members of a managed care health plan. You will perform Uniform Core, Functional, and Comprehensive assessments, documenting findings in GuidingCare and handing off to care managers.
This field-based role requires you to reside within commuting distance of the Massachusetts territory, travel up to 50% of the time, and maintain HIPAA compliance, with influenza vaccination required during flu
