Job description
Iowa Home Care Llc is seeking a Registered Nurse Case Manager to deliver compassionate, patient-centered in-home care. You will assess patients, develop individualized plans, and coordinate services with physicians and an interdisciplinary team to optimize outcomes.
Responsibilities include direct nursing care, documentation, patient and family education, and supervising home health aides. A valid RN license, CPR certification, and recent acute care experience are required.
