Job description
ProHealth Group is seeking a PRN Registered Nurse Home Health Case Manager for Bartow County, GA. You will provide skilled nursing visits in patient homes under physician orders and supervision of Clinical Services/Nursing Supervisor.
Responsibilities include initial evaluations, updating the plan of care, coordinating services, and documenting progress. On-call duties nights, weekends and holidays are assigned as needed.
