Job description
Your Health is seeking a Registered Nurse to join our growing primary care family as a Nurse Case Manager. You will manage a full patient load in the Grandstrand service area, performing essential duties to coordinate care and ensure patient comfort.
The role requires extensive home visits, care coordination, and documentation. Travel daily, with 7 on 7 off 12-hour shifts, and a salary-based full-time schedule.
