Job description
UnitedHealth Group’s Cape Fear Valley Hospice seeks a Registered Nurse Case Manager to deliver hospice care in patients’ homes. You will evaluate needs, develop Plan of Care with the team and coordinate treatment to support physical, psychosocial, emotional and spiritual well-being.
Responsibilities include conducting initial assessments within 48 hours, keeping accurate notes, and collaborating with the patient, family, and IDG. Travel within Harnett and Lee counties is required.
