Job description
Commonwealth Hospice, a part of LHC Group, is seeking a Registered Nurse Case Manager to deliver in-home hospice care in Virginia. You will assess patients, develop and implement the Plan of Care, and collaborate with the IDG to promote physical, psychosocial, emotional, and spiritual well-being.
Requirements include an active Virginia RN license, a valid driver's license and access to a dependable vehicle, a CPR certification, and 1+ year of clinical experience.
