Job description
TRU Community Care is hiring a Hospice RN Case Manager to provide compassionate, patient-centered care to hospice patients and their families. You will support patients in homes, facilities, and community settings, coordinating care and managing symptoms to improve comfort and quality of life.
The role involves regular local travel within the service area, collaboration with the hospice team, patients, families, and care providers, and ongoing documentation to ensure individualized plans of care
