Job description
Caretenders Hospice is seeking a Social Worker to provide in-home medical social services under physician direction and with the Interdisciplinary Group to support patients and families. The role includes travel within a 50-mile radius and collaborating on a comprehensive Plan of Care.
The position requires a Master’s in Social Work and a current Ohio Social Worker license, plus the ability to drive and access to reliable transportation. Hospice experience is preferred.
