Job description
Hueman is seeking Nurse Practitioners and Physician Assistants to conduct in-home Health Risk Assessments (HRAs) for Medicare and Medicaid members in and around Monticello, NY. The role offers flexible scheduling and training, with patient-focused care away from traditional clinics.
Qualified NPs and PAs must hold an active license, national board certification, and a current BLS. Experience with home-based assessments and documentation is preferred, and travel to patient homes is required.
