Job description
Source One Management Services, LLC seeks Nurse Practitioners or Physician Assistants for a flexible PRN role conducting in-home Health Risk Assessments (HRAs) for Medicare Advantage members in Rutland, VT. This position offers autonomy, an 8-day minimum per month, and competitive pay with incentives.
You will perform comprehensive HRAs, review medical history, medications, lifestyle and social factors, and travel locally to patients within roughly 60 minutes of home.
