Job description
Collage Home and Community Rehabilitation Partners is seeking an experienced Licensed Clinical Social Worker (LCSW) for PRN work in Fallbrook, CA and surrounding areas. You will assess, counsel, and directly support medically complex patients by developing individualized treatment plans and coordinating resources to improve adjustment to disability.
