Job description
Signify Health is seeking a Part Time Clinician (Nurse Practitioner or Physician Assistant) to provide In‑Home Health Evaluations, visiting patients in their homes and bridging gaps in care. The clinician will use an iPad with the Signify workflow and may conduct virtual visits as needed.
This part-time role offers 29.5 hours weekly with benefits and travel within licensed states. Active NP/PA licenses and board certification are required.
