Job description
Matrix Medical Network seeks a Nurse Practitioner to perform in-home Comprehensive Health Assessments (CHA) and care coordination for adults and seniors. You will review health history, medication adherence, and risk factors during visits at members' homes or SNFs, enabling improved care gaps and outcomes.
You will collaborate with PCPs, case managers, and other health professionals, delivering education, follow-ups, and targeted referrals while maintaining documentation per Matrix guidelines.
