Job description
Matrix Medical Network seeks skilled Nurse Practitioners to conduct comprehensive in-home health assessments. You’ll review health history, medications, and risk factors, then coordinate with physicians, case managers, and caregivers to close care gaps during visits at the member's home or in SNFs.
Strong NP licensure, prescriptive authority, and board certification are required along with BLS/ACLS. Flexible scheduling and potential FT opportunities support professional growth across regions.
