Job description
Matrix Medical Network is seeking NP-led clinicians to perform comprehensive in-home health assessments across the United States. Assessments focus on medical history, adherence, and social determinants to identify chronic conditions and close care gaps.
Providers work at member homes or skilled facilities with flexible scheduling Sunday–Saturday, 8am–7pm, and monthly earnings of a wide range based on visits completed. Ongoing education and licensure support are provided.
