Job description
Optum Home & Community Care in New York is seeking a Nurse Practitioner or Physician Assistant to perform in-home assessments and manage acute and chronic conditions. You will work with an interdisciplinary team to ensure high-quality, patient-centered care delivered at patients’ homes.
Daily travel to patient homes within the assigned area is required. Applicants should be licensed and certified for NY, with DEA licensure, and strong EMR skills.
