Job description
Monogram Health is seeking Advanced Practice Providers (NPs or PAs) to deliver high-quality in-home care to patients with multiple chronic conditions, traveling regularly to OH, IN, MO, KS, IL, WI, and MI. This field-based role involves comprehensive home visits, telehealth, and collaboration with an integrated care team.
The position offers a value-based, patient-centered model with competitive base pay, CEU/CME reimbursement, mileage, and generous PTO/holidays.
