Job description
Monogram Health seeks compassionate Advanced Practice Providers (NPs or PAs) to deliver in-home, evidence-based care to patients with multiple chronic conditions. You will join an integrated care team and support a value-based, population health model through home visits and telehealth.
Responsibilities include comprehensive assessments, patient and caregiver education, medication management, and coordination with PCPs and specialists.
