Job description
Monogram Health is seeking compassionate Advanced Practice Providers to deliver in-home, patient-centered care for adults with multiple chronic conditions. You’ll join an integrated care team and support telehealth visits, wellness checks, and comprehensive care planning.
Ideal candidates hold an NP or PA license with board certification, DEA, and at least 2 years of direct patient care. This field-based role requires travel to patient homes and reliable transportation to Louisville, KY.
