Job description
Grace at Home in Barrington, IL is seeking an Advanced Practice Practitioner (NP/PA) to deliver in-home and telehealth visits, collaborate with a multidisciplinary team, and manage complex chronic conditions for high-needs patients.
Reporting to the Clinical Manager, you will ensure quality care, accurate documentation, adherence to Medicare/Medicaid rules, and participate in quality improvement while traveling to patient homes.
