Job description
Signify Health, part of CVS Health, is seeking a Full-Time Clinician (NP/PA) to provide in-home health evaluations. Visits last about 45 minutes and include a medication/history review and physical examination.
You will use an iPad and a clinical workflow; no prescribing or lab orders are required. Role involves local travel to members' homes, occasional virtual visits, and delivering Diagnostic & Preventive Services as indicated by health plans.
