Job description
Monogram Health is seeking compassionate Advanced Practice Providers (NPs or PAs) to deliver in-home care for patients with multiple chronic conditions, including diabetes and CKD, in a value-based care model.
You will work with an integrated care team, providing long-term patient relationships in homes and via telehealth, with scheduled visits and no on-call obligations. Expect opportunities to grow clinically and participate in quality initiatives.
