Job description

Optum Home & Community Care is seeking an Advanced Practice Clinician (NP or PA) to perform annual in-home health assessments for Medicare Advantage and other plan members. This field-based role emphasizes education, care-gap closure, and collaboration with PCPs.

You will conduct comprehensive assessments, document diagnoses, educate patients about disease processes and medications, and address social determinants of health.
