Job description
Matrix Medical Network is a national in-home clinical network providing health assessments and care management. Providers conduct comprehensive health assessments in members’ homes or SNFs, reviewing medical history, medications, and social determinants to close care gaps and improve outcomes.
Opportunities offer flexible scheduling, competitive pay based on assessments completed, mileage reimbursement, 401(k), and referral bonuses. Sign-on bonus of $1,500 is available for new hires.
