Job description
Matrix Medical Network seeks qualified Nurse Practitioners or Physician Assistants to perform Comprehensive Health Assessments in patients' homes or Skilled Nursing Facilities. Each visit lasts up to one hour and focuses on medical history, medications, and overall health needs.
Providers work with PCPs and care teams, offering point-of-care testing and follow-up as needed. A strong emphasis is placed on mobility, licensure, and compassionate, patient-centered care.
