Job description
Optum Home & Community Care is seeking a Nurse Practitioner or Physician Assistant for HouseCalls in Jefferson County, NY. This field-based role offers flexible scheduling (8am–8pm across a 7-day week), with no on-call, no weekends, and no holidays.
You will perform annual in-home health assessments for Medicare Advantage and other plans, focusing on education, gap closure, and collaboration with primary care providers to improve outcomes and coordinate care across settings.
