Job description
UnitedHealth Group’s Optum HouseCalls team is seeking an Advanced Practice Clinician (NP/PA) to perform in-home health assessments for Medicare Advantage and other plan members. This field-based role focuses on education, gap closure, and collaboration with primary care providers to improve health outcomes.
The position offers flexible scheduling between 8am–8pm, 7 days a week, with no on-call, weekends or holidays.
