Job description
Optum Home & Community Care is seeking an in-home Advanced Practice Clinician (NP/PA) to perform annual health assessments for Medicare Advantage members and others. This field-based, non-prescribing role emphasizes education, gap closure, and collaboration with primary care providers.
Flexible schedules range 24–40 hours per week with no on-call, weekends, or holidays. Base pay plus bonus, comprehensive benefits, and driving to patient homes are included.
