Job description
Signify Health, part of CVS Health, is seeking a Part Time Clinician (Nurse Practitioner or Physician Assistant) to provide In-Home Health Evaluations. You’ll visit members in their homes and help bridge gaps in care, delivering personalized health assessments.
You’ll connect with patients in a flexible, nationwide network, perform medical history reviews and physical evaluations, and travel locally within licensed states as needed. Virtual visits are also part of the role.
