Job description
Signify Health, part of CVS Health®, seeks Part Time Clinician (Nurse Practitioner or Physician Assistant) to provide in-home health evaluations. Visits are typically 45 minutes in members' homes, with a focus on bridging gaps in care and connecting patients to the right services.
You’ll travel locally within licensed states, perform comprehensive reviews, and use an intuitive iPad workflow. This role supports virtual visits as needed and participates in a broad, nationwide clinician network.
