Job description
UnitedHealth Group is hiring an in-home Advanced Practice Clinician (NP or PA) to conduct Medicare Advantage assessments and close care gaps in patients' homes. This field-based role offers flexible scheduling and no on-call, weekends, or holidays.
As part of Optum HouseCalls, you will educate patients, document findings with PCPs, address social determinants of health, and contribute to an integrated care model aimed at improving health outcomes in the home setting.
