Job description
Signify Health, part of CVS Health®, is seeking a Full Time Clinician (Nurse Practitioner or Physician Assistant) to provide In-Home Health Evaluations. Visits last about 45 minutes and include med/health history review and physical evaluation, with optional diagnostic tests as ordered by the health plan.
