Job description
Signify Health, part of CVS Health, is seeking a full-time Clinician (NP/PA) to perform in-home health evaluations for members in their own homes. You will connect with patients, review medical histories, perform evaluations, and coordinate care using a streamlined iPad-based workflow.
Travel locally within licensed states is required and may include virtual visits when appropriate. Active, unrestricted licenses and board certification are required.
