Job description
Signify Health, part of CVS Health, is seeking a Full Time Clinician (NP or PA) to perform in-home health evaluations with patients in their homes nationwide. You’ll conduct comprehensive assessments, review medical history, perform physical evaluations, and coordinate with health plans to bridge gaps in care.
Candidates must hold board certification, be licensed to practice in the coverage areas, and be willing to travel locally.
