Job description
Signify Health, part of CVS Health, is seeking a Full Time Clinician (NP or PA) to provide in-home health evaluations to plan members across the country.
You’ll conduct visits with a tablet-based workflow, review medications and history, and coordinate with care teams to bridge gaps in care.
This role requires active licenses, board certification, and willingness to travel locally and within licensed states; weekly hours approx 40.
