Job description
Signify Health is seeking part-time in-home clinicians (NPs or PAs) to perform health evaluations at members' homes, using an iPad workflow. You will assess medical history, review medications, and conduct physical evaluations with the goal of bridging gaps in care.
The role requires active licensure, board certification, and multi-state licenses; travel is local and within licensed states, with some virtual visits and diagnostic services included.
