Job description
Signify Health, part of CVS Health, seeks a Part-time Clinician (Nurse Practitioner or Physician Assistant) to provide in-home health evaluations for members in their homes.
The visit lasts about 45 minutes and includes a medication/history review, physical evaluation, and possible tests per the health plan. You will use an iPad and may perform virtual visits, with travel within licensed states as needed.
