Job description
Signify Health, part of CVS Health, is seeking a Part Time Clinician (NP or PA) to provide in-home health evaluations for members in their own homes. Visits last about 45 minutes, including medication and history reviews, with a physical evaluation and optional diagnostic testing.
The role supports nationwide patient connections, travel locally within licensed states, and flexible in-home and virtual visits as needed. Part-time hours approx. 29.5 per week with benefits.
