Job description
Signify Health, part of CVS Health, is seeking a Part Time Clinician (NP or PA) to provide In-Home Health Evaluations for members in their homes. You’ll perform evaluations, review medications and medical histories, and help bridge gaps in care with health plans.
The role requires active licenses, board certification, and the ability to travel locally within licensed states. Approximately 29.5 hours per week; competitive part-time pay and benefits.
