Job description
Signify Health is seeking a Part-time Clinician (Nurse Practitioner or Physician Assistant) to conduct in-home health evaluations for members in the Watertown, NY area. You will assess health status, review medications, and coordinate care with health plans.
You will travel locally, perform assessments via an iPad, and may provide diagnostic services as required, without prescribing or ordering routine labs. Part-time role with flexible hours and benefits for eligible staff.
