Job description
Signify Health, part of CVS Health, is seeking a Part Time Clinician (Nurse Practitioner or Physician Assistant) to perform in-home health evaluations, meeting members in the comfort of their homes and bridging gaps in care.
The visit lasts about 45 minutes and includes a health history review and physical evaluation. You may travel locally and conduct virtual visits when needed, using an iPad with a clinical workflow.
