Job description
Signify Health, part of CVS Health, seeks a Part-Time Clinician (Nurse Practitioner or Physician Assistant) to perform in-home health evaluations across participating states. Visits last ~45 minutes and include history review, physical exam, and optional tests.
You’ll connect with members in their homes and coordinate with plans to bridge gaps in care. Travel locally within licensed states is required; virtual visits may be performed as needed.
