Job description
Signify Health, part of CVS Health, is seeking a Per Diem Clinician (Nurse Practitioner or Physician Assistant) to perform in-home health evaluations for members in their own homes. You will conduct visits lasting about 45 minutes, review meds and history, perform a physical, and potentially order diagnostic tests per health plan, without prescribing or altering treatment.
