Job description
Signify Health, part of CVS Health, seeks a Per diem Clinician (NP or PA) to deliver in-home health evaluations. Visits are ~45 minutes, with history review, exam, and optional tests, in members’ homes nationwide.
You’ll join a national clinician network delivering patient-centered care, with travel to licensed states and occasional virtual visits while supporting plan objectives and quality outcomes.
